Facilitates communication between sending and receiving facilities, including the use of telephone, computer software, and other communication and documentation tools. Documents a record of the encounter.
Responsibilities A day in the life of a Transfer Center Agent at Hackensack Meridian Health includes:
- Answers telephones in accordance with established policies and procedures.
- Utilizes professional tone and demeanor in all communication.
- Coordinates communication between sending and receiving facilities, and within the receiving facility.
- Accurately records details of all communication related to each transfer request in the electronic health record in an efficient and timely manner.
- Works in a collaborative manner with other members of the health care team.
- Demonstrates the ability to handle a high degree of pressure, heavy workloads, multiple requests, and numerous interruptions in a positive manner, establishing priorities for effective work completion. Able to diffuse challenging situations and respond appropriately.
- Demonstrates expertise in working with multiple computer applications as well as all functions of the telephone system.
- Escalates barriers and concerns using applicable policies and procedures as well as chain of command.
- Follows patient identification policies and procedures to ensure accurate patient identification.
- Utilizes downtime procedures for effective communication and documentation when necessary.
- Collaborates with Emergency Medical Services (EMS) to arrange appropriate and timely transportation based on established protocols.
- Obtains insurance information and communicates with insurance providers and/or case management and social services as needed.
- Initiates registration activities and completes registration as requested.
- Prepares handoff and other reports as directed.
- Maintains a clean and sanitary work environment.
- Other duties as assigned.
- Patient age specification: Neonate, infant, toddler, pre-school, school age, adolescent, adult, geriatrics.
- Must be able to sit and work at a computer with multiple monitors while speaking on a telephone for extended periods of time.
Qualifications Education, Knowledge, Skills and Abilities Required:
- High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
Education, Knowledge, Skills and Abilities Preferred:
- Emergency medical services experience
- Computer literacy
- Medical terminology
- Excellent communication skills
- Flexible and adaptable to change
- Ability to multitask
Licenses and Certifications Preferred:
- Emergency Medical Technician
- Paramedic
- AHA Basic Life Support

Starting Minimum Rate Minimum rate of $20.32 Hourly
